How to Begin Services

We review medical records, physician orders, and insurance information. Our team will work with you to understand the specific care needs of your loved one.

Service Authorization

We coordinate with your insurance provider or Medicaid to obtain service authorization. Our team handles all the paperwork and insurance communications.

Care Starts

Once approved, we match you with a qualified nurse or caregiver. Care typically begins within days of authorization, with flexible scheduling to meet your needs.

What You'll Need

To expedite your intake, please have the following information ready when you contact us:

  • Insurance Information: Medicaid, Medicare, or private insurance card and policy details
  • Physician Orders: Recent medical orders or prescriptions for home health services
  • Medical Records: Diagnosis, treatment plan, and any relevant medical history
  • Contact Information: Patient and family contact details, preferred communication methods
  • Care Needs: Specific services required (trach care, ventilator, G-tube, etc.)

How Long Does It Take?

The intake process typically takes 7-14 days from initial contact to the start of care, depending on insurance authorization. For GAPP services, approval usually takes around 30 days.
